An independent chair for HAZOP, HAZID, node review and overpressure studies. The person holding the pen does not carry the schedule, does not report to the project, and has no commercial interest in how many actions the study raises. Findings are recorded as found.

Why the chair being independent changes the output.
A study chaired by someone inside the project is chaired by someone who will be held to the date it finishes. That is not a comment on anyone’s integrity; it is a structural fact, and it shows up in how quickly a difficult node is closed.

What an independent chair does differently
The difference is mostly in what does not happen: nodes are not rushed, actions are not softened into observations, and a disagreement is recorded rather than resolved by seniority.

How it is delivered
Chaired, recorded, and carried to close-out.
A study that raises actions and never closes them has cost money and bought nothing. The engagement can end at the session or run through to close-out.
The session
Preparation against the document set, the session itself, and the record. Conducted in person where the study warrants it, remotely where it does not.
Travel is quoted separately, and ImmersaEngineering will advise where attendance is not justified.
Close-out
Actions tracked against a disposition taxonomy that distinguishes resolved and verified from implemented but unchecked, from risk accepted, from superseded.
Only the first of those is an engineering resolution, and the record says so.
Typical findings
What an independent chair tends to surface.
Illustrative and non-attributable. Each is invented; no engagement lies behind any of them.
- A node closed on an action that was never verified. The action had been implemented and signed. Nobody had rechecked whether it did what the study assumed it would do.
- A risk accepted without a named acceptor. The register recorded the acceptance. It did not record who held the authority to accept it, which meant nobody did.
- A deviation nobody wanted to open late in the programme. Raised at a point where the honest answer required rework. An internal chair carrying the date would have found it harder to hold open.
- A study whose attendance did not cover the scope. Operations were not represented for the nodes that most depended on operating practice. Recorded as a limitation of the study rather than left implicit.

Read one before you commission one.
The specimen is a process design review rather than a safety study record, so the subject differs from the work on this page. The structure does not: ranked findings, stated assumptions, residual risk, and a revision record that closes each finding against a named response.
It carries the assumptions relied on, a sensitivity table, the items examined and found sound alongside those that were not, the residual risk that remains after the recommended actions, and a passage setting out where the conclusion could be wrong. Every page is marked SPECIMEN; the facility, the client and the findings are invented.

A short description of the problem, the document set or the decision is enough to confirm suitability, propose a structure and give an indicative cost. A mutual non-disclosure agreement is executed before technical detail is exchanged.
